Case Management

A case manager is a professionally qualified individual who works alongside you in a therapeutic capacity, working in a supportive and innovative way, so that you can undertake routine activities and negotiate future possibilities. Our case management service provides co-ordination of the full range of services that have been assessed to meet individual need.

Following an initial face-to-face assessment, a written report will be produced that provides detail on our recommendations. Costings are also included in this report. Case management can assist with:

  • Providing a central point of contact and overview
  • Support and advice for family and carers

  • Devising, maintaining and developing care and support packages
  • Commissioning of services (including the recruitment, training and supervision of support staff)
